Waiver and Release of Liability for Luxury Dental Tourism

Acknowledgment of Risks

I understand that participating in dental procedures as part of a dental tourism package involves inherent risks, which may include, but are not limited to, injury, illness, complications, or other health-related issues that may arise before, during, or after the dental treatment.

Assumption of Risk

By using the services of Luxury Dental Tourism, I voluntarily assume all risks related to the activities and treatments I will receive. I understand that these risks may include, but are not limited to, complications from dental procedures, potential delays or changes in travel plans, and the overall uncertainties associated with international travel and medical care.

Release of Liability

I hereby release and discharge Luxury Dental Tourism, its directors, officers, employees, and agents from any and all claims, demands, and damages of every kind and nature, known or unknown, arising out of or in any way connected with my participation in the dental tourism services provided by Luxury Dental Tourism. This release includes, but is not limited to, liability for any injury, loss, or damage sustained by me due to negligence, breach of contract, or any other act or omission by Luxury Dental Tourism.

Medical Fitness

I confirm that I am medically fit to travel and undergo dental treatment, based on medical advice. I accept full responsibility for obtaining adequate travel health insurance, including coverage for medical treatment abroad and any necessary medical clearances required for travel and dental procedures. I acknowledge that it is my responsibility to inform Luxury Dental Tourism of any pre-existing medical conditions or concerns that may affect my ability to receive dental treatment or travel.

Agreement to Terms

By proceeding with Luxury Dental Tourism services, I agree to the terms outlined in this document, acknowledging that this waiver forms part of the terms of service. I understand that this agreement is binding and that I have had the opportunity to seek independent legal advice before agreeing to these terms.

I confirm that I have read, understood, and agree to this Waiver and Release of Liability.
